1. Payment in advance by personal check (at least two weeks prior) money order or visa.

2. You are responsible for providing health coverage and insurance for yourself and your dog if attending from out of province. Your are responsible for ensuring you and your dog have no health problems which would hinder you or you dog from participating from course content.

3. You are responsible for maintaining control of your dog at all times. The Dogmen Inc. assumes no responsibility or liability for loss or damages to property or persons caused by the above mention applicant or their K-9 at any time.

4. You are responsible for providing necessary documentation required for yourself and your K-9 to cross an international border. We are located in Ontario, Canada, one hour from the Buffalo International Airport. For an U.S. citizen this is photo I.D. and current rabies for your dog. The majority of our clients are American, please contact us if you have questions regarding this crossing.

5. The applicant agrees that the above information is true and correct to the best of their knowledge. The applicant agrees he or she has read and agrees to the terms of this agreement and those listed in our brochure. Submission of this application, is acceptance of these terms.
